COLLECTION: COLLECTION OF PERSONALLY IDENTIFIABLE INFORMATION ("PII") BY CONCARE, INC.
General. Concare, Inc. collects personally identifiable information ("PII,” or information like your full name, email address, mailing address or telephone number) when you voluntarily submit the information to us to participate in activities like surveys, polls, activities on our web site, or because you want us to furnish you with products, services or information. On the rare occasions when we obtain PII from a third party, we use only reputable sources of such information.
Cookies. Cookies are small bits of information that the Concare, Inc. Internet site places on the hard drive of your computer. Cookies remember information about your activities on our site and enable Concare to make your visits to our site more enjoyable and productive. If Concare, Inc. obtains PII from a third party, such as a business partner, our use of that information is governed by this Policy and such information will be used only for the purpose for which it is submitted, unless other uses are disclosed at the time of collection and you allow us to use your information in these other ways.
Concare, Inc. may share your PII with companies that are affiliated with us, Concare, Inc. may also share your PII with companies that are not affiliated with Concare, Inc., but would like to send you information about their products and services. If you do not want your name included on our mailing lists or lists shared with third parties, please “opt out” by checking the appropriate box when it appears on your screen. Additionally, you may send a letter requesting to be removed from our mailing lists or lists shared with third parties. Concare, Inc. employees, agents and contractors must have a legitimate business reason to obtain access to your PII. Concare, Inc. may share your PII with Outside contractors or agents who help us to provide you products and services, or to manage other activities that involve the collection of PII, but they may only use your PII to provide Concare, Inc. with a specific service and not for any other purpose.
If Concare, Inc. obtains PII from a third party, such as a business partner, our use of this information is governed by this Policy. There may be instances when Concare, Inc. may disclose PII without providing you with a choice in order to: protect the rights of Concare, Inc., or our employees, agents and contractors; to comply with legal requirements, such as a court order, and to protect the safety and security of visitors to our web site; to protect against fraud or for risk management purposes; or to comply with the law or legal process. SECURITY: CONCARE, INC. PROTECTS THE SECURITY OF PII
Concare, Inc. uses reasonable administrative, technical, personnel and physical measures to safeguard PII in its possession against loss, theft and unauthorized use, disclosure or modification. In addition, Concare, Inc. uses reasonable methods to make sure that PII is accurate, up-to-date and appropriately complete.
